CASHA provides the Online Service Directory to assist the public in locating practitioners within our professions. It is divided into two areas: Agencies and Private Practice.

• Information for Agencies will be submitted by one person per agency, which should be the owner, director, supervisor, or department head. There will be one listing per agency.

• Information for Private Practice will be submitted by individual private practitioners. It is important to note that all information submitted will be published for public viewing on the website, including any information which may include home (as well as business) addresses, phone numbers, cell phone numbers, and other personal identifying information.

In order to be certain that all members who submit information for the Online Service Directory understand and agree to these terms, we will require the submission of an acknowledgement/release form. This will provide us with a confirmation of the Online Service Directory information, as well as the release and consent to publish the submitted information on the website for unrestricted public viewing.

Those who wish to submit information for the Online Service Directory are asked to complete the acknowledgement/release form and mail it to:
